PCAHA GAME REPORT PROCEDURE
Referee Write-up Procedure:Â In all circumstances in which the referee is required to report violations of the rules (including Match Penalties, Gross Misconducts, Game MisÂconducts, 10-min. Misconducts in the last 10 minutes of the game, and teams refusing to start play) or any other matters of concern, he or she shall take the original (white) copy of the gamesheet and forward it within 24 hours of the completion of the game to the BC Hockey or PCAHA Officer responsible, together with a written report of the cirÂcumstances (on the reverse side of the gamesheet or attached).
ALL GAME REPORTS
A new procedure has been adopted by BC Hockey for the 2010-2011 season. All Minor Hockey game reports, including those involving Match Penalties and Gross MisconÂducts, are to be sent to the respective PCAHA Managing Director (see below). Do not send game reports to the BC Hockey District Directors. All game reports involving l Match Penalties, l Gross Misconducts, l Rule 10.14 (RefÂusing to Start Play), l Rule 1.13(c) (entering referees’ dressing room), l Game MisconÂducts (including Fighting and Checking From Behind), l 10-min. Misconducts in the last 10 minutes of the game, and l other matters of concern are to be forwarded within 24 hours of the game to:|

Note 1:Where infractions occur that must be reported to different PCAHA Managing Directors, the Referee shall forward the original (white) copy to the Managing Director dealing with the most serious infraction and forward a photocopy to the other Managing Director involved. Note 2:If a game report is faxed, the original game sheet and write-up must be forwarded by mail within 24 hours of the game. NO PERSON OTHER THAN THE ABOVE IS ENTITLED TO RECEIVE
A COPY OF THE REFEREE’S REPORT.
